The example employee mental health survey includes a menu of questions that considers both employee and employer responsibility for an individual's mental health and well being. You can mix and match the different question styles based on your ability to provide resources and support which differs greatly from employer to employer.

Ultimately the goal is to bring mental health out into the open, encourage employees to seek out resources, and find a way to support employees without undue hardship.

Why Implement an Employee Mental Health Survey?

Mental health is now at the top of strategic and executives' agenda and employers are investing more in supporting employee mental health through workplace mental health training initiatives, employee benefits, employee resource groups, and other programs.

An employee mental health survey is a good way to learn about your employees' perceptions and experiences. An employee may not yet feel comfortable sharing their mental health status with you in person, but may be willing to share if anonymity and confidentiality are guaranteed.

Your business can measure how perceptions are changing over time by comparing year over year responses to connect the dots between mental health and employee engagement. You can incorporate some of the mental health questions into employee surveys you already conduct or create a survey specifically for this purpose.

Before you Begin...

Mental health and illness can be sensitive topics, particularly in  businesses that don’t have an open-door history, strong communication and trust, or no track record of explicitly talking about or promoting mental health.  When exploring and implementing mental health metrics, ensure that you :

  • Encrypt and protect the employee mental health survey and its responses.  The individual employee must not be identified with a unique employee survey link. Doing otherwise will skew your results because employees will not trust that the responses are anonymous or can be tracked back to them.
  •  Use any data collected only for the purposes you communicated when sending out the survey. Do not exploit the data for any other purpose.
  • Use the data to make changes. If you are not prepared to make changes based on the survey results, do not conduct one! You'll do more harm than good. Conduct an employee mental health survey when you have the resources to action any changes.
